Dr Katz feels it is important to get a yearly check up for everyone, this includes blood work.  We currently use Quest Diagnostics but we can send your labs to any lab you prefer.  Please call us at least 24 hours before you are going to go to a different lab.

If you are of Medicare age and have Medicare, we do ask you to come in 20 minutes prior to your appointment to fill out Medicare Wellness forms.  Also please bring a list of your current medications that you take with all doctors, and a list of any specialist that you have seen in the past year. 
If you would like to print out the Medicare forms and fill them out prior to your visit please do so below. If you fill this out prior to your visit you do not need to be 20 minutes early to your appointment.
